For example, an individual with a $40k income and 2 kids would only pay 37% of total cost, with a $500 deductible. Another example, with 2 incomes over $100k and 2 kids, $1200/mo in prescription costs, the plan would pay 40% so they only pay 60%. The only requirement is to fill out a simple form and file your taxes annually.
